The Employee Health Nurse will be conducting the Annual Health Screen for all Physicians at the Medical Staff Meeting on Friday, September 9, 2011.

PPD vaccines will be given to those how qualify and the necessary paper work must be completed.

Please if possible, provide documentation of your Immunization Records.

Documents Needed:

• Updated PPD Record
• Completed Hep B Series or Declination

2011-2012 Influenza Vaccines will be available.
